STEP 1
Complete your forms online
Fill out the first four forms below: New Patient Intake, Oswestry Low Back Disability Index, RAND 36-Item Health Survey and Neck Pain and Disability Questionnaire. After a car crash, also complete the Motor Vehicle Accident Questionnaire. We’re notified as soon as they’re in, and we’ll call you to schedule your first visit. Prefer the phone? Call (801) 562-1531.
STEP 2
Consultation & history
We listen first: your symptoms, your goals, your health history and, after a crash, how it happened.
STEP 3
Measured evaluation
Posture analysis, digital range of motion and standard questionnaires. Imaging on site when clinically necessary.
STEP 4
Care begins
Once we have determined the best approach, we start some form of care on your first visit.
STEP 5
Review of findings and your plan
Your second visit is your review of findings visit. We review your findings with you in plain language. Together, we decide the approach you want to continue with, short-term care or a planned long-term program, with re-measurements so you can see your progress.

What will it cost?
We review costs with you before care begins, so there are no surprises.
